Most chronic care happens between appointments
It is one thing to know what would help your health. It is another to make it work alongside family, work, fatigue, symptoms and changing routines.
Your healthcare team continues to determine your clinical priorities and treatment. Our role sits in the space between those appointments, supporting the everyday behaviours, decisions and routines that help you carry agreed recommendations into daily life.
Each week, we help you focus on what matters most, decide what feels manageable and work through anything making the plan difficult to follow.
Practical support, built around the care you already have
Each week, you join a 60-minute live online group session designed to help you translate your care plan into practical action.
Rather than trying to change everything at once, we help you understand the priorities you are working towards and identify one or two actions that are realistic for the week ahead. Together, we can look at what has been working, what has been difficult and what may need to change.
Support may include food and daily routines, movement and energy, stress and sleep, planning, motivation, habit change and the practical barriers that can make following a care plan difficult.
The aim is not to create another plan alongside the one you already have. It is to help you make better use of the plan already developed with your healthcare team.
When life changes, the approach can change with it
Managing a chronic condition rarely follows a straight line. Some weeks are easier than others, and a strategy that works in one season of life may become unrealistic in another.
Weekly support creates an opportunity to notice those changes earlier. If an action has been difficult to maintain, we can look at why. It may need to become smaller, more specific or better suited to your current capacity.
Sometimes the challenge is not knowing what to do, but finding a way to do it consistently among competing demands.
The purpose of the program is not perfection. It is to help you keep working with your care, thoughtfully and practically, over time.
Their situation may be different.
The pattern may be familiar.
One person's relationship problem may reveal something about your own boundaries.
A conversation about procrastination may surface fear or perfectionism you recognise in yourself.
The details change.
Some of the underlying patterns can repeat.
Why ongoing support matters
Research into chronic-condition self-management suggests that support is more effective when it extends beyond simply providing information. Approaches studied in primary care commonly include personalised action planning, ongoing follow-up, monitoring of progress, problem-solving, feedback and support in managing the psychological and practical demands of long-term conditions.
The Mind Manual Chronic Care program has been designed around these same practical principles: clarify what matters, translate it into action, notice what gets in the way and continue adjusting the approach as life changes.

Led by someone who understands both sides of chronic care
Chronic Care is delivered by Melinda King, whose experience brings together counselling, behavioural health, health and wellness coaching, nutrition and functional medicine, alongside more than 18 years working within general practice.
As the owner and director of a high-volume general practice, Melinda understands both sides of chronic care: what the clinical team is trying to achieve and what patients are being asked to carry once they return home.
That perspective shapes the program. Recommendations need to remain clinically appropriate, but they also need to survive contact with work, family, stress, energy, habits and the ordinary unpredictability of life.
The work begins there.
